Posaconazole (Noxafil) causes liver failure ?

During this cycle of Vidaza (for treatment of AML), was administered Posaconazole (Noxafil, oral medication, thrice a day in full stomach) as a preventive anti fungal therapy. The 7-day cycle itself went off without incident. This was the first time he has been administered Noxafil.

After a couple of days of completion of the 7-day cycle, he developed fever/chills and so he was put back on Noxafil and IV antibiotic Imipenem was added. After about 5 more days on Noxafil, his liver bilirubin counts started going up and he showed signs of jaundice. Immediately Noxafil was stopped and so was the IV antibiotics and replaced with oral amoxicilin antibiotic.

However every day his LFT tests reveal worsening bilirubin (now upto 15.0) and lowering albumin leading to ascites/edema. He is on albumin transfusion for the last 5 days after stoppage of Noxafil with no sign of liver recovery.

His LFT and Kidney tests have always been excellent for the one year over which he has been suffering from MDS/AML. The doctor feels that he has liver failure since it has not recovered 7 days after stopping all medication which can harm the liver.

Anyone can share their thoughts/experiences with Noxafil and liver problems ?
