I was really lucky that my employer was really understanding and let me take a 6 month leave of absence for my transplant, and then I worked 4 days a week for my first 10 months back.

I don't think you really need to disclose details on your resume/interview - say that you were on medical leave, but stress that it was not a chronic health problem that shouldn't impact your ability to work with them in future.
