Celgene, the makers of Revlimid, have a patient assistance program called
RevAssist. They require healthcare providers to register for the program in order to be able to prescribe the drug to their patients. The forms for providers and patients are provided on the website.
There are several other
medication assistance programs listed on our Medical Resources page that might be of help to you.
