Dacogen

I am starting Dacon today. I have a port so it will be easier thatn poking me every day. I will be getting an IV for an hour a day, for 5 days..

I am also on Exjade 2500 mg daily. I have had 74 units of PRBC in a 2 year period.

I have MDS RAEB low grade.
Question: What can I expect in the way of results, side effects? Has anyone here had treatment with Dacogen? I have read quite a bit on the subject, and have talked with the Company, I just want to hear what other have to say..

dacogen or not????

My dad 72, tried vidaza 4 cycles and it didn't work. We are just on supportive care now and he recievies 2 pts blood every two weeks. Doctor mention Dacogen today but she doesn't really recommend it. I have read side effects. My dad's hmb 8 pltlts 33 white 1.5. He maintains blood count at 8 or 7.8. His pltls ae coming up slowly. What should we do? He has had about 35 transfusions and I know iron overload is coming soon. He says he feels good just gets short of breathe. Do we go to a doctor that has tried dacogen. Our current dr. isn't using it. ????

Dacogen after Vidaza treatment

Hi dee,

I think 4 cycles with Vidaza may be too few - as far as I understand you sometimes don´t see a response before 6 cycles but perhaps the adverse effects were prominent so the treatment couldn´t continue.

There are studies that show that Dacogen may be effective when there is Vidaza resistance :

"Leuk Lymphoma. 2008 Apr;49(4):690-5
Activity of decitabine in patients with myelodysplastic syndrome previously treated with azacitidine.
Borthakur G, Ahdab SE, Ravandi F, Faderl S, Ferrajoli A, Newman B, Issa JP, Kantarjian H.
Department of Leukemia, M. D. Anderson Cancer Center, Houston, TX 77030, USA.

Azacitidine and decitabine are the two hypomethylating agents approved by the Food and Drug Administration for the treatment of patients with myelodysplastic syndrome (MDS).

The efficacy of one agent post-failure of the other is unknown. Fourteen patients with MDS post-azacitidine failure/lack of response/intolerance were treated with decitabine.

Overall three patients achieved a complete remission, and one patient had hematologic improvement, for an overall response rate of 28%. Of the responders, one stopped prior 5-azacitidine owing to disease progression, two for no response and one for severe skin toxicity.

Grade 3-4 drug related side-effects were minimal. Global methylation studies in patient samples showed decrease of methylation after treatment with decitabine. As in our previous studies, there was no difference in hypomethylation between responders and nonresponders.

We conclude that clinically significant responses with decitabine can be seen in patients post-azacitidine failure without significant toxicity."

Then histone deacetylation inhibitors like Zolinza have showed good result in MDS but Zolinza is not approved for MDS (yet!) .

69 yo, dx MDS Interm-1 May 2006, transfusion dependent, Desferal 4 days with transfusion, Neupogen 2 injections/week, waiting for Eltrombopag/Promacta to be approved (last platelet count 42), asymptomatic
