What Cytogenetic Category
I was diagnosed with MDS (RCMD) in October of 2014. My bone marrow biopsy at that time indicated my cytogenetics as 46,XY,t(7;18)(q11.2;q12.2)[2]/46,XY[19]. A bone marrow biopsy in May of this year indicated my cytogenetics as 46,XY,del(7)(q22)[3]/46,XY[17]. With regard to the International Prognostic Scoring System, Revised (IPPS-R), is my cytogenetic category now based on having both chromosome abnormalities (t(7;18)(q11.2;q12.2) and del(7)(q22)) or just the most recent biopsy?

Typically the BMB reports tell you what you currently have in terms of cytogenetic abnormalities. You may want to see if there is additional language elsewhere in the report that use the language "in addition to" Based on what you wrote, it looks like the list of abnormalities is probably complete, so basing your IPSS on the current report seems appropriate, although it is probably unusual for translocations like you had in the earlier report to disappear. My inclination is to use the current data to assess IPSS.

MDS RCMD w/grade 2-3 fibrosis. Allo-MUD Feb 26, 2014. Relapsed August 2014. Free and clear of MDS since November 2014 after treatment with Vidaza and Rituxan. Experiencing autoimmune attack on CNS thought to be GVHD, some gut, skin and ocular cGVHD. Neuropathy over 80% of body. |

Thanks for your thoughts. I have an appointment with my 2nd opinion doc later this month. I will ask him what he thinks. Several people have been surprised at the translocation in the first biopsy. My wife is convinced it was an error. Maybe she is right. Thanks again for your thoughts. Data
