aggressivenes of mds

How can one identify that mds has became aggressive or converted to AML ?my father has again became transfusion dependent after 10 months of independency and is down with severe fever. Doctors is saying that the fever probably is due to mds getting agressive. Onky urine culture shed organism growth but even after antibiotics the high grade fever is sll there and with greater intnsity. Al this startd after startng tevolade. Maybe it is due to revolade or may be t was due to happen. Can we atart decogen even when he is having low platelets and is down with fever , as dacogen is th last resort. He is presentl neding platelets every alternate day

Father age 64 diagnosed with MDS RAEB-2, with 15%-18% blasts in October 2010. Only had blood and platelet transfusions. Ayurvedic treatment which showed result for arnd 5 months. Started Tahlidomide 100 mg started on 22 nd April 2011. Revolade 50 mg started on 2 nd april 2012.

Hi Neel,
As far as I understand it is difficult to know if the infection depends on more aggressive MDS or if all counts are lower due to the infection.

In any case the doctor has to find effective treatment for the infection - that can be a problem but hopefully they will find a kind of antibiotic that will help your father.

I have read much about Revolade (Promacta) but can't remember anything about low counts, infection and fever.

The doctor will probably wait with Dacogen - that drug like Vidaza should be taken during several cycles before most patients respond. Both drugs often decrease all counts initially.

Hope they can treat the infection effectively!
